Six analysis frameworks on one problem, in one artifact.

What it's for
RCCA runs six methodologies in parallel on the same problem — an 8D Report (D0–D8), DMAIC, 5 Whys, a real SVG Fishbone (Ishikawa) diagram, a real Pareto chart with an 80% “vital few” line, and a Corrective Actions summary — so every stakeholder reads the method they trust.
When to use it
Mandatory after any field failure, production escape, or customer complaint in regulated industries.
How it works
- Describe the problem; answer the 5-Whys clarify questions. → A short elicitation Q&A grounds the analysis, then all six frameworks generate at once.
- Switch frameworks via the tab strip. → 8D shows D0–D8 accordions (with team/containment/action tables), DMAIC shows its phases, Fishbone draws the 6M Ishikawa diagram, and Pareto draws bars + a cumulative line with the 80% threshold.
- Turn on Follow-on mode and edit — add/edit causes, sub-causes, actions. → CRUD is follow-on-gated (disabled controls say “Enable follow-on prompt to edit”); the Fishbone and Pareto redraw live, and items get NEW / MODIFIED / DELETED badges (with Undo).
- Fill the Corrective Actions tab and Export. → The Actions table (ID / Action / Type / Priority / Owner / Due / Status) is the closeout; export walks all six frameworks to PDF, stamped PDF, CSV, TXT or JSON.
